- Name of Service/Program/Initiative: Adult Services - Home Based Program
- Service Description and Eligibility Requirements, If Any: The Adult Services Program provides services to adults so that they can remain in the least restrictive environment for as long as possible, thereby avoiding more costly, and more restrictive, institutional placement. These services are often provided as stabilization for the adult who has been found to require adult protective services. The Adult Services Program provides case management, home-based services, adult foster care, day care for adults, nutrition, transportation, and counseling. Preadmission screening for nursing facility placement and assisted living facility assessments are provided.
- Service Preauthorization Required: Yes - The Uniform Assessment Instrument is utilized if a case is opened for services.
- Target Population/s: The elderly and persons 18 and over with a disability.
- Method of Administration: Local: 121 local departments of social services
- General Availability of the Service Across the Commonwealth (e.g., statewide, only available in certain localities): Available statewide
- Number of Persons Served Annually (Provide and indicate most recent year available): Close to 6,000 persons received home-based services last year through 3.6 million hours of service; 8,200 nursing facility preadmission screenings were performed; 1,100 assisted living facility full assessments and over 10,000 "short" assisted living facility assessments completed.
- Annual Service Cost/Allocation (Provide and indicate the most recent year available): Expenditures of $154 million were made at the local level last year. This includes federal, state and local match funding.
- Other Information or Comments About the Service: These services are closely tied to adult protective services as they are utilized to stabilize a person who requires protective services and assure that the risk of abuse, neglect or exploitation is minimized.

- Service Description and Eligibility Requirements, If Any: The Adult Protective Services Program protects vulnerable adults from abuse, neglect and exploitation. The program provides for the investigation of reports of abuse, neglect, and exploitation of adults 60 years of age and over and of persons aged 18 and over with a disability. The program is universal access with no fiscal or resource eligibility requirements.
- Service Preauthorization Required: No: The Uniform Assessment Instrument is utilized if a case is opened for services.
- Target Population/s: The elderly and persons 18 and over with a disability.
- Method of Administration: Local: 121 local departments of social services
- General Availability of the Service Across the Commonwealth (e.g., statewide, only available in certain localities): Available statewide
- Number of Persons Served Annually (Provide and indicate most recent year available): Close to 11,000 reports investigated last year with 60% founded to require services.
- Annual Service Cost/Allocation (Provide and indicate the most recent year available): There is $1 million available for Adult Protective Services at the local level. This is 80/20 state and local money.
