Service Description and Eligibility Requirements, If Any: state funds are made available to support families with family members with DD and individuals with disabilities to attend training they need to better understand their disability and learn how to improve their quality of life. Application and requirements available through contact with Access Independence

4. Target Population/s: People with severe developmental disabilities and their family members
5. Method of Administration: State - VBPD through grant RFP:

3. Service Preauthorization Required: Yes - Screening to ensure participation of self advocates and parents with children who have disabilities.
4. Target Population/s: People with severe developmental disabilities and/or family members of people with severe developmental disabilities.
5. Method of Administration: State - VBPD
6. General Availability of the Service Across the Commonwealth (e.g., statewide, only available in certain localities): Statewide
7. Number of Persons Served Annually (Provide and indicate most recent year available): 25
8. Annual Service Cost/Allocation (Provide and indicate the most recent year available): $140,000

4. Target Population/s: People with severe developmental disabilities
5. Method of Administration: State - VBPD through grant RFP
6. General Availability of the Service Across the Commonwealth (e.g., statewide, only available in certain localities): Statewide
7. Number of Persons Served Annually (Provide and indicate most recent year available): 30 YLF served annually
8. Annual Service Cost/Allocation (Provide and indicate the most recent year available): $ 125,000
