Summary of Services, Programs, and Initiatives Provided to Virginians with Disabilities

DEPARTMENT OF HEALTH

  1. Name of Service/Program/Initiative: Care Connection for Children (CCC) Network of the Children with Special Health Care Needs Program
  2. Service Description and Eligibility Requirements, If Any: Statewide network of centers of excellence for children with special health care needs (CSHCN) provides leadership in the enhancement of specialty medical services; care coordination; medical insurance benefits evaluation and coordination; management of the CSHCN Pool of Funds; information and referral to CSHCN resources; family-to-family support; and training and consultation with community providers on CSHCN issues.
    Medical Eligibility:
    Children with special health care needs are those who have disorders that:
    1. Have a physical basis;
    2. Have lasted or are expected to last at least 12 months; and
    3. Produce one or more of the following sequelae:
      1. Need for health care and ancillary services over and above the usual for the child's age, or for special ongoing treatments, interventions, or accommodation at home or at school;
      2. Limitation in function, activities, or social role in comparison with healthy age peers in the general areas of physical, cognitive, emotional, and social growth and development;
      3. Dependency on one of the following to compensate for or minimize limitation of function, activities, or social role:
        1. Medications, special diet, medical technology, assistive devices or personal assistance.
        2. Age Eligibility: Birth to 21 years of age.
        3. Residency Eligibility: Virginia resident
        4. Financial Eligibility (Required only for those seeking assistance from the CSHCN Pool of Funds): The CSHCN Pool of Funds is designed for uninsured and underinsured families with gross family income at or below 200% of the Federal Poverty Level (FPL), or for families living in Northern Virginia at or below 23.3% of FPL, based on the Virginia Department of Health's Regulations Governing Financial Eligibility for Services. Northern Virginia, as defined by this regulation, means the area that includes the cities of Alexandria, Fairfax, Falls Church, Manassas, Manassas Park, and the counties of Arlington, Fairfax, Loudoun, and Prince William.
    4. Service Preauthorization Required: Yes. Preauthorization is required for the use of CSHCN Pool of Funds.
    5. Target Population/s: Children with special health care needs as defined above.
    6. Method of Administration: State: Virginia Department of Health for statewide administration. Blue Ridge, Roanoke Area, and Southwest Virginia CCCs are currently administered by the Virginia Department of Health. Negotiations are underway for them to be locally administered. Local: Virginia Commonwealth University Health System Authority administers Central Virginia CCC. Eastern Virginia Medical School and Children's Hospital of The King's Daughters administer Hampton Roads CCC. Inova administers Northern Virginia CCC.
    7. General Availability of the Service Across the Commonwealth (e.g., statewide, only available in certain localities): Statewide
    8. Number of Persons Served Annually (Provide and indicate most recent year available): 6,191 persons in SFY 2001
    9. Annual Service Cost/Allocation (Provide and indicate the most recent year available):
      $4,982,693 in SFY 2001
    10. Other Information or Comments About the Service: Covered services under the CSHC Pool of Funds are those that are medically necessary for diagnosis and treatment of a covered condition. They include the following:
      1. Durable medical equipment (limited to eye glasses, contact lens, prosthetic eyes, hearing aids, nebulizers, and orthopedic and urology appliances.)
      2. Diagnostic testing (laboratory, imaging and audiology procedures)
      3. Therapies (physical, occupational, and speech)
      4. Medications
      5. Hospitalizations
      6. Dental orthodontic and prosthodontic appliances for children with maxillofacial conditions

  1. Name of Service/Program/Initiative: Child Development Clinic (CDC) Network of the Children with Special Health Care Needs Program
  2. Service Description and Eligibility Requirements, If Any: Statewide network of clinics provides diagnostic assessments and care planning, follow-up care coordination and referral. Each clinic professional team consists of a physician, nurse, social worker, educational consultant and psychologist. Consultations are available from other pediatric specialists as needed. The professional staff also offers consultation, training and advocacy throughout the community, focused on promoting the healthy growth and development of children.
    Medical Eligibility:
    Children suspected of having developmental and behavioral disorders.
    1. Age Eligibility: Birth to 21 years of age
    2. Residency Eligibility: Virginia resident
    3. Financial Eligibility: Payment of charges on a sliding scale based on the income level and size of the family as described in the Virginia Department of Health's Regulations Governing Financial Eligibility for Services.
  3. Service Preauthorization Required: No
  4. Target Population/s: Children suspected of having developmental and behavioral disorders.
  5. Method of Administration: State -- Virginia Department of Health. Local -- District health departments administer ten of the clinics. Other -James Madison University administers one clinic.
  6. General Availability of the Service Across the Commonwealth (e.g., statewide, only available in certain localities): Statewide
  7. Number of Persons Served Annually (Provide and indicate most recent year available): 2,865 persons in SFY 2001
  8. Annual Service Cost/Allocation (Provide and indicate the most recent year available):$3,414,855 in SFY 2001
