Am I a stakeholder in the MFP Project?

You are a stakeholder in this project if you are:

• Living in a nursing home, an intermediate care facility for individuals with mental retardation (ICF/MR) or a long-stay hospital and thinking about, or planning to, transition to the community;
• A family member or caregiver of an individual who is thinking about or planning to transition;
• A public or private provider of nursing home, ICF/MR or long-stay hospital care or services;
• A public or private provider of services, supports, transportation or housing in the community for individuals with disabilities or seniors in need of services;
• A disability or aging advocacy organization; or
• A representative of local, regional or state government. 

Virginia is committed to the full inclusion of all stakeholders in the planning and implementation of this project and highly values your active participation in the success of individuals transitioning to the community from nursing homes, ICFs/MR and long-stay hospitals.  Ongoing collaboration and communication among stakeholders is crucial for the project’s success.
