Am I a stakeholder in the MFP Project?

You are a stakeholder in this project if you are:

• Living in a nursing home, an intermediate care facility for individuals with developmental disabilities (ICF-DD) or a long-stay hospital and thinking about, or planning to, transition to the community; Have resided for at least 90 consecutive days in a hospital, nursing facility (any days spent in short-term skilled rehabilitation services do not count towards the 90 days), intermediate care facility for individuals with intellectual disability (ICF/MR), long-stay hospital, institute for mental disorders (IMD), psychiatric residential treatment facility (PRTF), or a combination thereof;
• A family member or caregiver of an individual who is thinking about or planning to transition;
• A public or private provider of nursing home, ICF/MR or long-stay hospital supports; Have resided for at least 90 consecutive days in a hospital, nursing facility (any days spent in short-term skilled rehabilitation services do not count towards the 90 days), intermediate care facility for individuals with intellectual disability (ICF/MR), long-stay hospital, institute for mental disorders (IMD), psychiatric residential treatment facility (PRTF), or a combination thereof;
• A public or private provider of supports, transportation or housing in the community for individuals with disabilities or older adults in need of supports;
• A disability or aging advocacy organization; or
• A representative of local, regional or state government.
