Virginia Easy Access Launched
The Secretaries of Health and Human Resources and Information Technology have launched a new website that brings together resourcesone spot. Virginia Easy Access is a web portal that allows seniors, adults with disabilities, and their caregivers to access information about providers and other supports in the com for long term supports by allowing people to access them in munity. Housing, transportation, and emergency preparedness are also included. The web portal is supported by VirginiaNavigator and 2-1-1 Virginia. You can search for supports and service by zip code on VirginiaNavigator and call or e-mail questions to 2-1-1 to find out more information. MFP Housing and Transportation Resource Bank
An MFP Housing and Transportation Resource Bank has recently been added to this website. The resource bank includes information about how individuals can access housing and transportation, and the work that is being done to increase available, affordable and accessible housing and non-Medicaid transportation in the Commonwealth of Virginia.
DMAS announces the availability of Supplemental Home Modifications
DMAS is delighted to announce the availability of Supplemental Home Modifications for individuals participating in the Money Follows the Person (MFP) Demonstration. Individuals participating in MFP now have access to both prior to discharge home modifications that prevent the individual from transitioning and home modifications that exceed the $5000 Medicaid Environmental Modifications maximum limit, once the individual has transitioned to the community. A fact sheet containing information on the application process, as well as, the contact information of the five regionally-based Centers for Independent Living that will coordinate all requests is available here.

This single-fold, book style brochure provides an introduction and overview of the project including the eligibility requirements, services available, and who to contact for more information. Transition Coordinator Provider Agency List
Users may now dowload a list of the Medicaid-enrolled Providers for Transition Coordination. These providers assist an individual in developing a transition service plan to move from a facility to the community using the Elderly or Disabled with Consumer-Direction Waiver.
Operational Protocol Approved; Project Begins July 1
Virginia's Money Follows the Person Demonstration Operational Protocol (Project Guidebook) was approved by the Centers for Medicare and Medicaid Services on Thursday, June 24, making it possible for the Project to begin as scheduled on July 1st. The Operational Protocol is available here.
"Transition Coordination: Nuts and Bolts" Training Presentation Now Available
Everyone who moves to the community under the Money Follows the Person Project will have assistance developing a person-centered transition plan. This in-person training was conducted at 5 sites across Virginia in July 2008 and the presentation is available here.
Money Follows the Person Appendix to DMAS Provider Manual Now Available
On June 27, the Department of Medical Assistance Services released the Money Follows the Person Appendix to the Provider Manual. This Appendix contains information about provider enrollment and pre-authorization, and includes details about the new and added waiver services.

Introduction of Money Follows the Person (MFP) Memo
The purposes of this memorandum are to notify providers of the Money Follows the Person (MFP) Program which provides individuals living in nursing facilities, intermediate care facilities for persons with mental retardation (ICF/MRs), and long-stay hospitals with informed choices and options about transitioning into a more integrated community setting.
